Event registration question planner

Start with attendee name and email. Add session choice when people choose a programme, dietary questions only when food is provided, and an optional way to discuss adjustments. Use this planner to leave out questions your event does not need.

Choose what your event needs

Your 4-question registration plan

  1. What is your full name? Required

    Identify the attendee.

  2. What email address should receive your event details? Required

    Send joining information and updates.

  3. Are there any adjustments you would like to discuss with the organiser? Optional

Offer a private follow-up without demanding medical details.

  4. Is there anything you would like to ask the organiser? Optional

    Resolve questions before the event.

This plan does not reserve places, collect payment or send confirmations. For hybrid events, review the dietary question’s conditional logic. Explain who reviews answers and when attendees hear back.

What changes between a webinar and a catered event?

A webinar needs a reliable email address for the joining link. A catered event may also need a dietary follow-up. A hybrid event needs attendance format so online attendees avoid venue-only questions. Do not require a phone number, employer or address unless it changes how you organise attendance.

Keep registration separate from marketing

This planner leaves out promotional consent. If you add marketing, explain that separate purpose and make the choice independent of registration. Ask for adjustment needs without requesting a diagnosis or medical history.
