Event registration form: questions + free template
An event registration form should collect enough information to organise attendance without asking attendees for unnecessary personal details.
Quick answer
Start with name and email, then add only event-specific fields such as attendance type, session choice, accessibility needs or dietary requirements. Explain how the organiser will use the information.
What should the form include?
- Attendee name
- Email address
- In-person or online attendance
- Session or ticket choice where relevant
- Accessibility requirements
- Dietary requirements for catered events
- Questions for the organiser
Ask only what the event needs
A webinar may need little more than a name and email. A catered in-person event may need dietary information. Add fields because they change how you organise the event, not because they are available.
Separate registration from marketing consent
Registering for an event does not automatically mean a person has agreed to unrelated marketing. If you want separate promotional consent, make that choice clear and distinct.
